Print the sheet, keep it next to your monitor, and write each reading down immediately with the time. Sit for five minutes first and use the same times each day. When the page is full, photograph it to turn it into a chart or bring it to your appointment.
Why does the sheet print the date format at the top?
So a date like 3/9 is never ambiguous later — for you, your doctor, or the scanner. Pick DD/MM or MM/DD to match how you write dates and the sheet labels the column accordingly.
